MCBDD is partnering with Discount Drug Mart to provide Flu vaccines to individuals ages 19 and up. See attached flyer and forms for more information.

Clinic Registration – REQUIRED
When attending the clinic you MUST:
1. Review the Flu Vaccine Information Sheet
2. Print, complete and bring a consent form for each person receiving a vaccine.
3. Bring your insurance card or information.
