Major Unusual Incidents (MUIs)

An MUI (major unusual incident) is an alleged, suspected, or actual incident where there is reason to believe the health and safety of an individual was adversely affected. Examples may include, but are not limited to: physical abuse, sexual abuse, verbal abuse, attempted suicide, death, exploitation, misappropriation, medical emergency, missing individual, neglect, unapproved behavior support, unscheduled hospital admission, and/or significant injury.

MUI reporting and investigations help everyone to identify why an incident occurs and how to prevent it from happening again.

Who Must Report an MUI?

    • All providers who are contracted, certified or licensed to serve persons with developmental disabilities
    • All Medina County Board of DD staff
    • Anyone paid to provide Medicaid waiver services

What if I suspect an MUI?

If you suspect an MUI, whether involving your family member, a person you serve, or someone else, you have a few options for how to proceed. You can call:

    • The DODD MUI Hotline (which allows you to leave anonymous tips)
    • Your/your child’s SSA
    • Our after-hours emergency cell

The important thing is that you let someone know so that the MUI can be investigated and the individual’s safety can be protected.
